The patient's condition or illness may be acute or chronic or the child may have had traumatic experiences or emotional distress, in addition to the one that brought the child to the hospital. The quality and amount of social support is also critical and inadequate care from parents or caregivers can compound the psychosocial stress a child experiences. Three primary sources of stress have been identified in pediatric patients: (1) separation from parents or caretakers through hospitalization (relocation to a new environment) (2) loss of independence and control which accompanies illness and hospitalization (3) fears and anxieties about medical procedures which may cause harm or pain, and worry about death (Golden 1984).

Although this dual aspect of art therapy has always existed, it has not been stressed in the literature, perhaps because we take it for granted. It is particularly relevant in medical settings. Whether time is of the essence as in an emergency or hangs heavy, as in long periods of recovery and rehabilitation an activity which is at the same time both diagnostic and therapeutic is especially helpful. Some of the chapters in this book focus on the use of art in assessment, while others stress the use of art in treatment.
